Specialized Physical Therapy

Outpatient physical therapy services are made available with an emphasis on an active approach to rehabilitation. Various modalities are available for treatment of acute and chronic conditions and all patients are instructed in exercises for improvement of flexibility and strength. Patients are educated and trained in progressive lumbar and cervicothoracic stabilization. All evaluations and treatments are provided by a licensed physical therapist, and all treatment plans are functional and goal-oriented. 

 

Stabilization and Conditioning Programs


The goal of spine stabilization training is to control back and neck pain while continuing activities of daily living. Stabilization training is a multi-dimensional program of education, flexibility, strength, coordination, and endurance training aimed at preventing repetitive microtrauma to spinal structures. It is an active patient participation program which empowers the individual to manage his/her back problems and prevent further injury. 

 


Manual Therapy Interventions

Manual therapy interventions are utilized primarily to increase mobility in restricted areas of the musculoskeletal system and to reduce pain. These "hands-on" interventions include a variety of soft tissue procedures and joint mobilization techniques. Incorporating specific movement patterns with these manual techniques will enhance maximal movement and function. 

 



Fibromyalgia and
 
Chronic Pain Program

These programs integrate posture realignment, stretching, trigger point releases, body mechanics education, and progressive fitness/aerobic programs specialized for fibromyalgia and chronic pain. The therapists work closely with their clients to help relieve or lessen many of the disabling effects of fibromyalgia and chronic pain.

 


Functional Capacity Evaluations

(FCE)

The FCE is a comprehensive, objective testing of a person’s ability in work-related tasks. The FCE measures specific abilities in material handling (lifting tasks), and non-material handling (positions and movements).  In addition, the worker’s degree of effort, performance consistency, and self-limiting behavior is assessed and documented.  The staff at Horizon Spine Rehabilitation has the ability to customize the FCE according to each particular case so that recommendations for return to work, further intervention, or case closure can be efficiently and effectively made. 

 

Headache and TMJ Program

This program is developed with a focus on decreasing the intensity and frequency of headaches and pain caused by TMJ (temporal-mandibular joint).  Utilizing manual therapy to improve and normalize the joints in the head, jaw and neck, we are able to decrease the need for medication and improve lifestyles.    

The headache program is most successful for cervicogenic and tension headaches.  It is also useful for migraine headaches and vertebral insufficiency headaches.  The techniques utilized are gentle and utilize no manipulative or “cracking” procedures.    

We have successfully treated several patients that have headaches for as long as twenty years.

The TMJ program uses manual hands-on treatment to improve the ability to open and close your mouth and decreases pain in the joint.  Again, we have treated people that have had TMJ pain for more the ten years with success. Click here to see video of this treatment procedure.

Spine Health – For Kids

The “Spine Health – For Kids” program at Horizon Spine Rehabilitation addresses both painful and non-painful spine conditions that are common among children and teens.  The staff at Horizon Spine Rehabilitation possesses advanced training in the area of spinal rehabilitation, athletic training, and strength and conditioning.  The “Spine Health – for kids” program consists of the following:

Evaluation.  A thorough history and examination will be performed by a licensed physical therapist.  During this examination, the child’s gait, posture, muscle coordination and standing balance, flexibility, and strength will be assessed.�

Education.  The education provided will be specific for each child’s needs. This education may focus on the potential causes of the child’s symptom, ramifications of poor posture, sport conditioning considerations, or a variety of other topics specific to the child’s condition or complaint.�

Home Exercise instruction.  Each child will be given a home exercise program to address the findings during the physical examination.  These exercises may consist of flexibility exercises, coordination exercises, and/or strengthening/endurance exercises.�

The staff at Horizon Spine Rehabilitation is committed to providing comprehensive spine rehabilitation to all ages.  Our “Spine Health – for kids” program is designed to evaluate, educate, and provide a continuum of resources to children and their parents concerning the management of spine conditions.

 

                              Frozen and Difficult Shoulder Program

The frozen shoulder or adhesive capsulitis program was developed utilizing the Niel Asher technique for frozen shoulder.  Please see www.frozenshoulder.com for more information regarding this program.  Although this program was developed for treating frozen shoulder, it has proven extremely successful for treating nearly all shoulder pathologies.  These include tendonitis (biceps, supraspinatus and rotator cuff) and bursitis (subacromial and deltoid), rehabilitation after surgery, manipulation under anesthesia and osteoarthritis.    
This technique has undergone several clinical trials and has shown significant improvements over traditional physical therapy for treating frozen shoulder.  This program requires fewer visits and is less painful than traditional therapy for this condition.  Click here to see a video of this treatment procedure.


Balance Intervention

The physical therapy staff at Horizon Spine Rehabilitation provides intervention addressing impaired balance, falls, and vestibular dysfunction.  Our clients will receive a comprehensive balance evaluation, including one or all of the following:  a musculoskeletal examination of the cervical and thoracic spine (neck and upper trunk), examination of the visual and vestibular systems, and/or examination of functional balance skills using researched based assessment tools.  Our goals for intervention include improving the client’s level of function, decreasing complaints of dizziness, and increasing safety at home and in the community. 

 

Multiple Sclerosis Program


The Multiple Sclerosis program at Horizon Spine Rehabilitation is designed to assist individuals with MS to achieve their optimal level of function. Our physical therapists work closely with referring physicians and nurse practitioners to establish a treatment plan at the time of initial diagnosis or during/after exacerbations. Physical therapy intervention may include any or all of the following: 1) outpatient sessions at our clinic, 2) fitness instruction on-site or at an individual’s health club, 3) home evaluations, and/or 4) functional capacity evaluations. Physical therapists work with patients to set realistic, functional goals, provide education regarding symptom management, identify the need for adaptive equipment and mobility aids, and become independent with a home exercise program.
 

 

Custom Orthotic Fabrication

Custom orthotics can be used to help correct over thirty common diagnoses related to poor foot function, including pathology in the hip, knee, and foot. Because foot function affects the entire alignment of bones leading to the spine, pain and dysfunction in the knee, hip, and low back often result if foot position is poor. Our custom-fit orthotics are fabricated to your foot, allowing the orthotic to restore normal foot positioning with greater effect than a common off-the-shelf orthotic. Restoring normal foot function aids your body in healing itself and often improves the effectiveness of other necessary treatment or therapies.

 

Decompression Therapy

The 3D ActiveTrac combines high force decompression with three-dimensional adjustment and optimal patient positioning to address the issues specific to each patient. The three-dimensional capabilities enable us to accommodate all patients and make it an excellent tool for correcting postural deformities. This state-of-the-art traction table allows us to successfully treat conditions such as: disc herniation, degenerative disc disease, degenerative joint disease, facet joint impingement, sciatica, sub-occipital headaches, and chronic back/neck pain.


The advantage of the 3D ActiveTrac is its ability to slowly apply a traction force to allow distraction of the spinal vertebrae. With a standard traction table, the relatively rapid speed-of-force application causes spinal muscles to reflexively contract, preventing maximum benefit of the traction. The slow rate-of-force production of the 3D ActiveTrac prevents this muscular guarding, and will therefore give you better results. It is a more comfortable and effective modality when compared to a standard traction table.
