Information for Physicians

Horizon Spine Rehabilitation is . . .

• Specialized. Treatments at Horizon Spine Rehabilitation are provided by physical therapists with either board-certification in Orthopaedics or Neurology, Manual Therapy Certification, or Doctor of Physical Therapy Degrees. These physical therapists all specialize in the treatment of musculoskeletal and neurological conditions.
• Exercise Based. Our patients are able to access our video library through the Internet. This capability fosters patient adherence by allowing the patients the opportunity to review their home exercises on video format to ensure their technique is correct.
• Efficient. Referring physicians will receive a typed summary within 48 hours of the patient's initial physical therapy evaluation and monthly progress reports prior to their physician appointments.
• Cost Effective. We participate with most managed care plans. Each physical therapy treatment will be provided by a licensed physical therapist, to ensure timely modification of the patient's treatment program and expedite the patient's return to maximum function.
• Convenient. Both of our Omaha locations are located between three large hospital systems and are easily accessible from the interstate systems. Our Blair location is located on the corner of Highway 75 and South Street. Convenient parking is available at all locations.

Scheduling Appointments
Initial appointments can be scheduled by phone by the patient or referral source. If appointments are arranged by the referral source, the following patient information is required: patient name, date of birth, diagnosis, and payment source. All initial evaluation appointments are scheduled for one hour. Patients are encouraged to come 15 minutes before their scheduled time to complete paperwork. Clinic hours are Monday through Friday from 7:30 a.m. to 6:00 p.m. Convenient, free parking is available.
